What is a GP chronic condition management plan?
It's a plan your GP makes with you to manage a chronic condition. It replaced the GP Management Plan and Team Care Arrangements on 1 July 2025 [1].
If you already had one of the older plans before then, you can keep using it for allied health visits until 30 June 2027 [2].
Who can get one?
Your GP decides whether your condition would benefit from a plan and from allied health care, such as physio [2]. If you think it might help, ask at your next appointment.
How many physio visits does Medicare help pay for?
Up to five allied health visits a calendar year [2]. That's five in total, shared across all the allied health practitioners you see. So if you see a physio and a podiatrist, the five visits are split between them. The count starts again each January.
Aboriginal and Torres Strait Islander people can have up to ten visits a year [1].
How much does Medicare pay?
From 1 July 2026, Medicare pays $63.40 for each physio visit under the plan, as long as the visit lasts at least 20 minutes [3].
I don't bulk bill. You pay my usual fee, and Medicare pays you back the rebate. Here's what that means for my fees:
- First visit at my Monday clinic, 60 minutes: $155. You're out of pocket $91.60.
- Follow-up at the clinic, 45 minutes: $135. You're out of pocket $71.60.
- Home visit, 60 minutes: $190. You're out of pocket $126.60.
How do I get a referral?
Ask your GP. If a plan suits you, your GP refers you with a letter [1]. The letter doesn't have to name a particular physio, so you can choose who you see [1].
The referral stays valid for 18 months from your first visit, unless your GP writes a different time on it [1]. Please bring the letter to your first visit.
What happens at the visit?
At the first visit I listen to your story, examine you, and explain what I find. We agree on what matters most to you, and you leave with a plan you can do between visits. With your permission, I keep your GP up to date.
You can see me at my Monday clinic in Peregian Beach, or I come to your home anywhere from Caloundra to Noosa, the hinterland and Gympie.
What if I've used my five visits?
You can keep seeing me privately, without a Medicare rebate. If you have private health insurance with extras cover, you may be able to claim from your fund. The fees and funding page has the details.
